GCH's new community outreach health initiative was created to address the growing health care needs within the community for those who are low income and uninsured (or underinsured).  Our goal is to provide prevention, treatment and the coordination of services that address heart disease and diabetes among GCH's communities, through the use of our mobile unit. The Health Coach provides intake, health screenings/assessments, health coaching, diabetes case management, community health resource referrals, health information, and patient care follow up.
