Visit the Sleep Disorders Center of Michigan, 35600 Central City Parkway, Suite 103, Westland, any Wednesday from 5 p.m. to 6 p.m. for a free mask fitting clinic.

This exercise program is designed to help you make an easy transition back to an exercise program, or help you to get started for the first time.

GCH's new community outreach health initiative was created to address the growing health care needs within the community for those who are low income and uninsured (or underinsured).  Our goal is to provide prevention, treatment and the coordination of services that address heart disease and diabetes among GCH's communities, through the use of our mobile unit. The Health Coach provides intake, health screenings/assessments, health coaching, diabetes case management, community health resource referrals, health information, and patient care follow up.

As a service to the community, GCH offers free blood pressure screenings for all ages.

Diabetes is a life-long health challenge requiring you and your family to take responsibility for maintaining good health.  The GCH Diabetes Self-Management Education Program is here to help patients effectively meet those challenges.  We use a team approach to assist you in the management of diabetes.  Team members include Registered Nurses and Dietitians.  Other resources available include physicians, exercise specialists, pharmacists, physical therapy and social work.
