LINKUP WITH THE HEALTH COACH
To expand the hospital’s mission, to improve the health care needs
of the communities we serve, Garden City Hospital (GCH) has developed
a community outreach initiative to address two of the most chronic health
conditions that currently affect its population: heart disease and diabetes.
OUR MISSION IN MOTION
To address critical health challenges in the community, GCH has created
an innovative community mobile health program, LinkUP with a Health Coach,
to provide free prevention and coordinated services to help reduce heart
disease and diabetes within our community. This outreach program utilizes
the Garden City Hospital Health CoachSM, a 40 foot motor coach/bus, to provide services to low income and uninsured
(or underinsured) populations in need of health care.
GCH’s Health CoachSM provides intake, health screenings/assessments, health coaching, diabetes
case management, community health resource referrals, health information,
and patient care follow up.
You must be 18 years or older to be eligible for the LinkUp program at
no cost, regardless of your insurance status.
REQUESTING THE GCH HEALTH COACHSM IS EASY!
The GCH Health CoachSM is available for health and community events in the surrounding Garden
City Hospital communities. Please allow a minimum four week notice for
your event. All events are subject to hospital approval.
